| Proof of completion of the University of Florida’s Master’s degree in Medication Therapy Management. | ✔ |
| Proof of completion of the University of Florida’s graduate certificate in Comprehensive Medication Management (CMM) | ✔ |
| Proof of completion of a pharmacy residency in MTM or Ambulatory Care Pharmacy. | ✔ |
